An accepted identity document may be requested.
Address evidence may be required when information cannot be matched.
Proof of participation may be requested when automated verification does not match.
Income documents may be needed for the household-income route.
Many applications can be checked without uploading every possible record. If the applicable verification process asks for proof, use the notice to identify whether it needs identity, address, program participation, household status or income information. The name, address and other details should match the application whenever possible.
Use a clear, complete and current document that shows the information requested. Do not crop out identifying labels, expiration details or the portion that connects the record to the applicant. If a document contains unrelated sensitive information, follow the secure submission instructions rather than sending it through the general contact form or ordinary email.
Gov iPhone's first-step intake does not accept document uploads. Wait for the authorized secure channel and contact Enrollment Support if the request is unclear. The Lifeline document guide explains common proof categories in more detail.
